Mission
The Aging and Adult Services Team provides services necessary to fulfill the legal mandate of North Carolina in assuring that adults who are disabled and/or aged and are fragile or vulnerable, are protected and receive services to remove the factors placing them in danger.

Services Include:

  • Adult Day Care/Day Health
Provides organized programming and activities during the day, to support adults’ personal independence. This is a state certified program providing individualized care for adults with disabilities and respite for their caregivers.
  • Adult Placement Services
Assistance to older adults or adults with disabilities and their families in finding suitable adult care home or nursing facility placement.
  • Adult Protective Services
This program strives to correct and prevent abuse, neglect, and exploitation of adults with disabilities.
  • Community Alternative Program for Disabled Adults (CAP/DA)
Services disabled adults who are Medicaid-eligible and qualify for nursing level of care. The program utilized Medicaid benefits to ensure that a person can remain safely at home and receive the services needed to avoid institutional care.
  • Guardianship
Evaluates individual situations to determine if a person with disabilities is in needs of an appropriate guardian to ensure that their needs are met and that they are safe from abuse, neglect or exploitation. The program provides all necessary services as a disinterested public guardian, appointed by the Clerk of Court under General Statute 35A.
  • In-Home Aide Services
Provides assistance with basic home managements tasks by contract.
  • Meals on Wheels
Preparation and deliver of nutritious meals to clients’ homes to prevent institutionalization or malnutrition.
  • Representative Payee Services
When appointed as a representative payee, the agency manages income to meet client’s needs when the client has been determined unable to manage financial benefits.
  • SA-In Home Program
Provides an alternative to placement in an adult care home for Medicaid eligible individuals who could live at home safely with additional support services.
  • Services for the Blind
Provides a variety of services for people who are visually impaired or blind.

Located at 157 Paragon Parkway, Clyde, NC 28721.
